General Practice Data for Planning and Research (GPDPR)

You have possibly heard or read in the press recently about NHS Data Opt out. NHS Digital are launching a new way of collecting data from GP Practices in order to support the planning of NHS services and for research purposes.

As a practice we are leally required to participate in the sharing of this data. We must also be clear and transparent with our patients about how their data is handled and shared. We want to alert you to this change to allow you to think about your options and consider whether you wish to opt out of this data sharing. If you wish to register a Type 1 Opt-out with your GP practice your data will not be shared with NHS Digital, this should be done by completing the form below and returning this to your GP practice either by post or email to gp.n81045@nhs.net . This form will then be held in your medical records. If you are unable to complete the form online you can request for a form to be sent out to you by calling 0300 303 5678.
