Just like online banking, you can look at your GP records on a computer, a tablet or a smartphone, using a website or an app.

If you want to register download to print the form PatientOnline Registration_form and bring into the surgery in person with two forms of Identification one must be a photograph ID (passport or driving license) and one proof of address (utility bill in your name) to complete the registration.
