To assist in the smooth running of the Practice and make the prescription process more efficient please post your requests in the following ways:
Tick the items required off your previous prescription form on the right hand side and drop this into the RED BOX situated just inside the main waiting room.
In writing, stating clearly the items required, printing your name and date of birth, signing and dating the request. This can then be dropped off into the prescription box provided or POSTED with a stamped addressed envelope. If you notice any error we would be grateful if you would inform the reception staff.
IF ABSOLUTLEY NECESSARY by telephoning the surgery on 0151-424-6221 but please ring after 11:30 am so that staff can deal with urgent appointments.
From time to time you may receive with your prescription a request to see your Doctor or Nurse. This is because most medications require reviews to ensure their greatest benefits.
Do you want to order your repeat prescription on-line?
You can now do this via Patient Access.This service is currently only available for patients aged 16 and over.
You need to register for this service with the Practice first – to get the relevant information please call into the surgery and bring some form of photographic I.D. such as your current passport or photographic driving licence.
